Thinking back to the Season 5 finale, I remember being quite convinced that the sounds emanating from the Swan core, as the Incident was happened, were far too similar to Smokie to be coincidental.
I believe The Incident, or the newly-revised Incident, created Smokie. Perhaps Smoky was a byproduct of the Island trying to protect itself from future catastrophes - like an antibody. It makes sense that something illogical, or at least something requiring a suspension or leap of logic, took place at the confluence of the Swan core imploding and a hydrogen bomb detonation.
As Juliet was very much in the mix when this occurred, it follows that she somehow was transformed or evolved into Smokie. Not unlike the origins of Watchmen's Dr. Manhattan or Valiant Comics' Solar Man of the Atom.
This would shed some much-needed light on the still-puzzling scene from late Season 4 when Smokie took snapshots of Juliet and Kate.
Though it is implied that Smokie heals young Ben in the Temple in 1977, it is never shown or explicitly mentioned. Similarly, no one in 1977 Dharma ever mentions Smokie. This leads me to conclude it's because Smokie does not yet exist.
Here's an interesting twist: Perhaps in the original timeline someone else - like Phil or another of Radzinski's lackeys - was at ground zero of the Incident, and became Smokie.
In the new timeline, with Juliet becoming Smokie, maybe we will see a smoke monster that is white, instead of black, and righteous, instead of calculating. Given the color inverse of title screen, such a plot twist is highly plausible.
