Ok, so this might seem a little weird, but just stick with me, because I think that it could be very true. If you do not believe that whatever happened, happened, then you will not believe in this theory. Ok here goes!
So Daniel just died, and Jack knew about his plan. So I believe Jack will try and carry through with Daniel's plan, and this will cause the incident. Him causing the incident, will cause the button to be put into use, Desmond will fail to push it and their plane will crash. In other words, this has always happened, Jack doesn't know it because he hasn't live it yet. So Jack is at fault for ultimately crashing the plane.
Also a theory on 4,8,15,16,23,42:
They are useless numbers that Hurley sees on the Hatch (Swan), he then ultimately leads to a broadcast of the numbers, either he does it, or someone else does it. Then that Sam Tooney fellow hears them, believes they are cursed, Hurley finds out about them uses them to win the lottery and thinks they are cursed. They are really only just numbers on the side of the hatch that are useless. He just doesn't remember reading them, because it hasn't happened for him yet.
