Smokey did not create Alex. Probably. Lots of people on LOST see dead people, so Alex was probably whatever manifestation of the Island or Jacob or whatever that "creates" dead people.
Leaving out on-Island interactions with Christian, who seems to be everywhere and seen by everyone now... on-Island he's been seen by Frank, Locke, Sun, Claire, Hurley (maybe), Jack, etc.
SEERS of OTHER DEAD PEOPLE:
Hurley (Ana Lucia, Charlie, Eko--maybe, playing chess--and whoever was in the cabin when he peered in the window)
Jack (Christian, off-Island)
Kate (Claire, off-Island)
Eko (his brother Remi)
Michael (Christian, on the boat, and Libby, though it could have been a dream, the hospital room was different)
Ben (Alex)
Who am I missing? (I left out Locke/Boone and goopy hallucinogens, and Charlie seeing angels.)
In any case, most these appearances seem to be beyond Smokey's reach, or nature... if they are all caused by the same force. And I'd like to think that the show hasn't created two forces that just happen to make dead people appear and talk to living characters. So whatever put Charlie at Santa Rosa is probably what put Alex in front of Ben.
MILES:
Miles can't see dead people but does talk to them. Maybe he developed this skill as a baby on the Island. But while he doesn't seem to see dead people (except on-Island Christian) he gets a lot of information out of the dead bodies he comes across. His skill doesn't seem dependent on any external circumstance, though in some cases a hand-held vacuum cleaner is needed.

SAWYER: Well, who was she with?
MILES: She called him "Dad". I'd have followed them, but I have a restraining order.
DESMOND and CAESAR:
Who do we know has seen the time-travel-teleportation "flashes" other than those flashing? Desmond (when Faraday flashed away) and Caesar. Caesar said he say a bright light (though it could have been the sun, when night turned to day) when people disappeared; he also said other on the plane saw people disappear, but I don't believe he said they saw the flashes too. So was this a continuity error in the writing (having Caesar see it) or was pre-shotgunned Caesar "special" like Desmond?
REASONS for RETURNING:
Who has a reason for coming back to the Island? Sun (to find Jin), Ben (whatever his reason is), Kate (to find Claire), Hurley (probably because Charlie told him to, but we don't know yet). Jack has a half-baked, if enthusiastic, reason (to "save everyone") while Sayid had no choice.
It seems likely, though, from the last episode that Illiana and company have a reason for going to (or returning to?) the Island. Wonder what it is... I laughed when I read the theory that Widmore himself is inside the big metal case. Kudos to whomever thought that up.Theory by Kirbyjon
