First time theory poster:
Kate is pregnant. She and Jack knocked boots the night before boarding Ajira Airlines flight 316 (adding to the whole recreate Oceanic Airlines flight 815 conditions...Sayid was the captive passenger substituting for Kate, Kate was the pregnant passenger substituting for Claire).
After "WH,H" we know that Sawyer chooses Juliet and that Jack and Kate have failed repeatedly as a couple. In the same episode we see Kate and Roger meeting and having a connection which I believe will strengthen with Kate's efforts to save Ben. I believe that Ben (however changed) will return to the DI compound and continue to be raised by Roger and his new lady fiend...Kate.
Kate's pregnancy will progress but at delivery she will die (due to the island curse...which is initiated by the impending Incident?). For whatever reason, Roger will in part blame Ben for her death (whether Ben causes the Incident or interferes in some way in the effort to save Kate)as evidenced by this conversation between Roger and Ben in the Dharma van just prior to the Purge (from "The Man Behind The Curtain" transcript:
BEN: Do you really blame me?
ROGER: What?
BEN: Do you really think it's my fault that she died?
ROGER: What do I know...
[Ben looks at his watch]
ROGER: Why do you keep looking at your watch? You got a date? [Pauses] Listen...if it makes you feel any better, I will do my best to remember your birthday next year.
BEN: I don't think that's going to happen, Dad. [starts to unzip bag]
ROGER: What do you mean?
BEN: You know, I've missed her too. Maybe as much as you have. But the difference is, for as long as I can remember, I've had to put up with you. And doing that required a tremendous amount of patience.
I believe that Roger and Ben were talking about Kate and not Ben's biological mother, Emily. Roger previously stated that he blamed Ben for killing Emily (same episode), so why would he now say "what do I know..." if refering to Emily. I think they're talking about Kate.
What will be the ramifications of Kate's death? I don't know. Will one of the Losties try to go back in time via the FDW and try to change the past to keep Kate alive?
It seems apparent that we will see time shift at least once again when Widmore is tricked into leaving the island. How will that effect events?

According to the transcript, in "The Man Behind The Curtain", Ben and John have the following exchange near the end of the episode:
BEN: You know, John, you're not wrong. Some of the things I've told you—some of the things I've told everybody—are simply not true.
LOCKE: Like what?
BEN: Well, for starters, I wasn't born on this island.
So...either Ben remembers some of his history despite Richard's vague "he will not remember any of this" in "WH,H" or he thinks he's lying to John during this exchange.
