The theme of pregnancy, birth, and death is a mystery central throughout all five seasons of lost. So here is the real reason for every pregnancy and the deaths related to it.
In "Lafleur", Amy and Horace conceive and birth a son in 1977 on the island. This is the first time this has happened in the show's history. So why was this different from the other pregnancies that Juliet had to watch fail in the second trimester? Its not because some event hasnt happened yet, like Sawyer suggests. It is because Amy did something different in her pregnancy. What she did was carry a necklace of an Ankh, that her husband Paul was wearing when he died. This Ankh is a symbol of eternal life in ancient egyptian culture. We also know that the four-toed statue on the island carried an ankh in each hand before it was destroyed.
Paul was an other who knew about the power of the ankh in pregnancy and he told his wife before he was killed in "Lafleur". This is the reasdon his body was taken by Richard Alpert, because he used to be one of his people before he switched to the Dharma Initiative. Amy took the ankh and was able to conceive and give birth on the island with Horace. The statue was of Taweret, the god of pregnacy and childbirth, which is fitting for an island where pregnant women die. Taweret had hippo ears, long straight human hair, big breasts, and an ankh in each hand. The only thing we don't see is the chest of the statue.
This means that if the mother carries an ankh with her during her pregnancy on the island, she will be able to give birth (Amy). If not, her uterus will age and she will die (Juliet's patients, Ethan's wife). The only way to prevent this is to leave the island as soon as possible (Sun), or to come to the island very late in pregnancy (Claire, Rousseau).
Juliet was brought to the island by Ben because she delivered a baby on the island in 1977. Ben's love Annie died in childbirth. He thought Juliet was able to prevent this and she looked like Annie. This is why Ben stole Alex from Rousseau, because he couldn't have children of his own. This is also why he protected Karl, the son of Horace and Amy. Their son was born in 1977, making him about 27 in 2004 when 815 crashed. Karl was conceived and born on the island and that made him special to Ben. So he took an infant girl named Penny from her D.I. parents in 1974. Penny's sister was named Charlotte, born in 1971 or 1972.
