The whole premise of the show is called Lost, because it is Jack who is lost. He had a nervous breakdown and is in an isolation chamber in a mental ward. The airplane crash was Jack crashing.
The numbers all correspond to the ages of his patients that died under his care. It was him, not his father that killed the last patient due to his drinking. To him, his father is dead because it was his father that turned him in. Jack could not handle the hospital termination and his divorce. He then drank himself into a hypnotic stupor. At the beginning of each show, we see Jack’s eyeball. That is his hypnotic stare. While in the isolation chamber ( the island) his made up characters come to life. All his characters are a figment of his mind. They are actually his “7 deadly sins” come to life.

Anger – Michael/Walt and Ana Lucia
Greed – Sawyer and Eko
Sloth/Apathy – Sayid
Gluttony – Hurley
Lust - Kate (sexual) and Charlie (drug/alcohal)
Jack has lots of time to make up backgrounds for all his characters. There are also many others around who are real people that he may remember in his past that add to his stories.
In order for Jack to come back to reality, all his seven deadly sins must be tamed and changed for the better or be destroyed. In the first season finale, he had overcome his anger (Michael & Walt, greed (Sawyer), and some pride (Jin), but in the opening of season two, they came back (on the boat, except for Walt (little anger). In the 2nd season finale, he has gotten rid of his anger (Michael/Walt/Ana Lucia). Claire’s baby is seen as a new beginning. Jack is not ready to bond with the baby( never seen him holding or caring for the baby) until his demons are tamed. I think he will be left with the baby after they all the seven deadly sins are demolished or saved. The doctors are the Others. They are real doctors monitoring his vital/psychological signs, that is why they are the Others. They are trying to bring him back, but he does not want to come back to reality. He wants to remain lost in his made up world. That is why he retreats to the cave, the hatch, anywhere but staying in plain sight on the beach where he can be rescued. The 108 mins signify his timely shock treatments. When the hatch blew up, that signified his departure from the isolation chamber and now the doctors are going to work on each of this demons individually. That is why they requested the five individuals to go with them.
Lost is about forgiveness, learning how to control your demons, and allowing others help.
