The island is the land of the dead
Jacob is one of the Gods of the underworld. Either Osiris, Anubis or Hades
The Black Smoke Monster/Man in Black is either Cerebus the guardian of the land of the dead and servant of Hades or one of the other above mentioned gods of the underworld
Pretty cool concept if you ask me. Don't get me wrong though, just because I think this might be the land of the dead doesn't mean everyone on the island is dead as well. They're all alive for the most part I think, with the possible exception of some of The Others.
I'm not going to go in depth as to why I think this is so. Anyone who remembers seeing the depiction of The black smoke monster being summoned by Anubis on the wall beneath the temple in Dead is Dead and the events that took place in The Incident should understand why I believe this to be so.
