Upon consideration of Jack's plan to "put things back the way they were", it is clear that he believes that flight 815 will land in LA, like it was supposed to. But he forgets to think of some of the effects it may have on a few other parties involved such as the others, and the dharma initiative.
If flight 815 were to land in LA, our losties would have never been on island to travel back to 1974. This means, that if Jack's plan works, then he never would have been there to blow up the bomb which obviously would kill everyone on the island.

If Jack's plan does work, as he blows up the bomb he kills everyone on the island. The conciousness of our losties, from what we are led to believe, would jump to them getting off of flight 815 safely in LA. This is because the hatch would have never been built etc. So what would happen to the others, or the Dharma initiative?
I believe this is what the incident is.
Jack blows up the bomb, killing everyone on the island. But since they correct things so that 815 lands safely, then what happens to the consciousness of the dharmas/others?
I dont really know where to go from here. I'm thinking that maybe a second after the bomb goes off the dharma/others reappear intact on the island. Maybe there is evidence of an explosion. Or maybe they just have memories of dying in that explosion some how. Possibly some of them grasp a few faces or names of our losties from their memories. This may explain why Ms. Hawking knew she shot her son Daniel. It didn't actually happen but she has some memory of it.
