Jacob was 'created' in The Incident (or a similar one)and now simultaneously exists at every point in time on the island.
Ever read Watchmen or sen the adaptation? If you haven't, there's a character called Dr. Manhattan- an ordinary guy who after an accident in a research facility is torn apart but continues to exist at a quantum level, with the laws of physics as his play toy.
Jacob has a similar existence, but spread out over the space time continuum of the island.
At some point during the Dharma Initiative's existence, an ordinary man (maybe someone we've met, maybe someone we haven't. Jacob Oldham, perhaps? Long shot, anyway...) gets caught up in an accident involving the 'energy' that powers the Orchid's time-space traveling ability. In the process, 'Jacob' was torn apart and his presence or spirit or soul etc. was spread out across space time. He is confined to the Island, but par of him exists at every second of the Isand's past, present and future.
This is how Jacob always knows the 'right'' path for the eople on his Island. The Smoke Monster is either a cloud of the aforementioned
'energy' that is either controlled by Jacob or the only physical form he can take.
By 'scanning' people, Jacob's monster is able to see their past, present and possible future, and from those memories, he is able to 'save' a kind of 'copy' of them if you will, a 'memory' under his control or answerable to him given physical form. Christian, Yemi, Kate's Horse, Ben's mother and recently Alex are all 'copies' of them, extrapolated from memories and kept in stasis from time.
This is also how Jacob/ The Island 'heals'-restoring a person's body to a previous state of health. It can't make anyone physically 'better' than they have been previously, can restore prior states of health, saved from the past.
This is how Jacob 'rewards' according to his whims, he let's his followers contact a part of their dead loved ones, and maybe even uses his time-space powers to prevent aging and death- perhaps not aging anymore was Richard's reward for services tended and orders carried out for Jacob.
'But we know the smoke monster existed before Dharma because of the Hieroglyphics'. Aha- like I said, following the incident that gave him time-space control, Jacob now existed everyWHEN on the Island. Upon being turned into his current state, Jacob exists in 1977,but also in the thousands and also 1000s of years ago among the first people who settled the Island, who- because of his ability seemingly bring the back the dead- believed Jacob and/or The Monster to be avatars of the God Anubis, who helped deliver pharaohs and priests safely to the underworld for judgment. Although his 'birth' would occurred ages later, Jacob now existed to wheedle and order the Island's first dwellers, who built the temple and the statue in his honour.
Of course, being spread around time that much leaves Jacob a little incorporeal, so he often needs surrogates to help him stay balanced' and make life easier for him. Plus, all the recent 'flashes' in time may be making Jacob's existence a little uncomfortable- all the commotion and Swan explosions and Incidents and Donkey Wheel turning may be a danger to Jacob and by extension The Island and the fabric of space-time.
The surrogates, such as Christian, have some measure of free will or at least free thought, they ultimately answer to him because he's keeping them 'alive'. Claire may be becoming one of these, Chrisitan is one of these, and Locke might be one too now- part of the 'smoke' that contains all the memories of The Island and the people who've lived there, controlled by Jacob- the man confined to the past, present and future of The Island.
