In the last episode (316), Ben leaves Jack after they meet with Ms. Hawking. He says he has to "meet with a friend." Then, the next time you see him on the plane, he's a bloody wreck with a cast over his arm.
I think he saw Desmond, knows that's her boyfriend, and figured she had to be in town, too. Which, apparently she was, because in "Jughead," she tells Desmond that she'll go with him to Los Angeles to meet with Daniel Faraday's mother. In that same episode, Widmore is very concerned for his daughter's safety and tells them to continue hiding out.
I think he knew this might be his last chance to exact some revenge on Widmore for killing his daughter, had a little time to kill, so he figured he'd take a shot at it. He might have even tailed Desmond out of there.
He probably got in a big, bloody battle with Desmond and Pene. And, unfortunately, I think he probably did kill Pene, or maybe their son, Charlie.
This would bring Desmond back into the fold. Because now he'll want to get his revenge upon Ben more than anything else, so then he'll be willing to go back to the island. How else are they gonna bring back Desmond to the island when he's tied down by Pene? And what else would Ben be doing at the point that causes him to be so wounded?
The only other explanation I can come up with is that he got into a battle with Sayid (apparently they had a big falling out) and that's why Sayid's being escorted by the guard on the plane. Theory by evan
