This is based on another theory but has been tweaked.
I belive that most people on the island are already dead and that is why they can't get off the island. They died in the plane crash and are living in new bodeys that the island made for them. Not everyone survived the crash as only the ones with a strong qill to live did and mayby Desmond not pressing the button increased the healing powers. If you are in a new body then you can't leave the island.
The Alive list (these people did not die in the crash)
The Oceanic 6, Michael and Walt.
If you can get off the island then you are alive.
Claire. To give birth I'm guessing you have to be alive, mayby she died in her house.
Not sure list.
Bernard. Even if he is alive I would guess that he would stay around with Rose.
The Dead list (these people are dead and are living in new bodies)
Locke. This is why he was healed he had a new body and why he is not off the island.
Rose. Again she was healed by the island by being put into a new bodey.
Jin. This is why he had sperm again.
Sawyer. This is why he is not off the island and why Kate is trying to help him.
All other Background survivors as they didn't get off the island.
