My theory is that JACK died in Sydney and came back to life when the plane crashed on the Island.
Think back to the opening scene of Episode 1: Jack is flat on his back on the ground, his eyes are closed, he's wearing a suit and tie (burial attire), he looks dead; then suddenly his eyes open as if he just woke up from the dead.
When Jack finds the coffin on the Island that allegedly had his father in it, there is no body inside.
Flash-forward to the not-so-distant future (based on Jack's age) - Jack is trying to fill a prescription signed by his father, and while arguing with a surgeon in the hospital where Jack practices, he mentions his father's office is right above them. Twice he refers to his father in the PRESENT tense.
In the future Jack mentions that he goes over seas every weekend hoping the plane will crash. He wants to go back to the Island and it appears the only way he thinks he can is if he is dead.
In Present day Jack doesn't know he was dead and came back to life yet; but my theory is that Locke does know this, and that's why he keeps ambushing Jack's attempts to leave the Island.
Jack could've had a drug and alcohol problem when he went to Sydney and died there, but when he crashed on the Island, he came back to life as a changed person. His father didn't leave Sydney in a coffin, Jack did. His father wasn't even on that flight.
