I believe that when the plane crashed the perceptions of the passengers on board changed. If they were unconscious and dying like Boone for instance, his life would have flashed before his eyes, also if he heard his sister Shannon coughing, his mind might flash to a scene and his urgency to "get her an inhaler" or vice versa, Shannon's mind, being unconscious and having trouble breathing may go to a dreamlike state interpreting reality in this way. I think every episode will have a correlation to what is actually happening in reality.
Charlie is dying that is why Desmond, who is really a Doctor trying to save him, keeps telling him he is going to die…
Sawyer, Jin, Michael & Walt ended up in the water, and are in the process of being saved when one of the rescuers says "We're going to have to take the boy… first" Flash to Michael waking up in a hospital room. "Where's Walt? Where's my boy?" "He's in the pediatric ward with all the "other" children".
People have been littered around and not easily found. Kate finds the key and actually escapes from the dead agent only to come upon the focal point of the wreckage and other victims helping Jack & Sawyer until she faints from her injuries. Jack wakes up after seeing his father who tells him that he is proud of him and he has more work to do. He helps the EMTs get the victims to a nearby hospital - The Hatch
Jin has lost the child she was carrying while Doctor Juliet fights to save her life - she would have died if Juliet did not take the baby.
Several of the character's are in comas - they wake up from their unconscious states and altered perceptions (what we’ve been shown) and then see their reality and how it inspired their "misinterpretations."
Keep in mind this could also just be an Island in the Bermuda Triangle where scientists have discovered a way to keep the skies and ocean safe from the magnetic currents that have caused so many problems. Since Desmond failed to push the button and caused the crash, all this is true reality on a base located in the Bermuda Triangle.
