We've known for some time that "The Others" have a flair for theatrics. They dressed as abandoned jungle dwells when they had clean clothing and shelter all along. They staged an entire decoy village to fool Michael, and consequently, Sayid. They even tricked Sawyer into thinking that his heart would explode if he didn't behave himself.
For this reason, I have come to suspect that Carl is not as innocent as he may seem. Nobody really knows the circumstances behind him being in the cage opposite Sawyer in the first episode. His escape attempt seemed a bit half-hearted, and we don't know that he was actually beaten by Tom or any of the others. It's also hard to tell if loud techno music and a bunch of odd images could really brainwash somebody.
So what could Carl be? He could be the one that Ben and others in authority intend to finish Ethan's work in compiling a "list" of fuselage survivors. Ethan failed to do so, having been discovered as a fraud due to Hurley's census. The list given to Michael from Mrs. Klugh seemed to have been entirely focused on getting Jack to remove Ben's tumor - not whatever the initial objective was. So how do "the others" finish compiling a list after revealing their hand?
Convince Sawyer and Kate that the young man they are taking back to their beach is a victim and needs to be helped. Put him in a cage before Sawyer wakes up. Place some blood under his nose after a lame escape attempt. Put him in a chair with an IV drip, loud music, and weird pictures. I doubt Alex would really even know what he is up to, in fact, Carl could be going along with a charade to prove to Ben that he belongs and is worthy of being a companion to his adopted daughter. Alex having more of a conscience than the others could be a hindrance, but could also be used to Ben's advantage if Kate and Sawyer are so thoroughly convinced that they're helping someone who is against their adversaries.
"The Others" like to use complicated measures to accomplish simple deceptions, and it is unlikely that a group of people as determined as them would stop trying to obtain their "list" just because Ethan was killed and Ben met an obstacle in his spinal tumor.
Somebody has to determine who the real "good people" are among the core group of crash survivors. Why not someone who is seemingly in a position of needing their help?
