I have a feeling the DR (Brooks) at the mental institute is the man behind the whole thing. In his office we see the pic of the island that they are stuck on, on the wall behind him... and it seems likely that he could have had contact with each of the 'losties' through their connections to a hospital.
There are still some that i cant figure out yet... i might be forgetting somethings or they might not have been revealed yet.
