Because turning the wheel, you exit into the OTHER polarity universe. (thus why you end up at the ANTIPODE of the planet from where the island is, which is Tunisia.)

But for those with Ben, that left the island via boat, (and didn't end up at the antipode) well, I wouldn't be surprised if some of them were able to get sick and not be healed by this island.
