Radiation wreaks havoc on a woman's reproductive system (hence the "old-looking" x-rays Alpert shows Juliet when trying to recruit her as well as the difficulty Juliet's siter had in getting pregnant following radiation for cancer treatment), often making it impossible to conceive or carry a child to term. [As a side note, this may explain why most people don't get cancer on the island and Rose's cancer was cured.]
