Those Goddamn Writers!
Hurley is what really throws a kink into everything. Hurley sees dead people. Exclude Miles since he doesn't actually see apparitions. How are we to determine what is a legitimate apparition or Man #2 and/or Smokey?
I think when we get to the heart of it, this is what the writers and producers want us to pine over all summer.
Here is what I think:
Hurley is special. Period. Either he was born that way or Jacob made him that way (Jacob called it a blessing, which implies natural anomaly or literally a blessing that he gave to him)
So, I think pre- and post- island visions are legitimate. After all, have we seen proof that Man #2 has appeared off island?
So my first claim is that Dave was not Man #2. That, I believe, is still to be resolved. (Actually I'm on the side that thinks he was Libby's husband, and the 'disaster' that caused his death also caused Hurley's ability and Libby's breakdown.)
Who else can we throw onto this boat? Charlie, Ana-Lucia, and anyone else I'm not thinking of that appeared SPECIFICALLY to Hurley.
For the Medusa spiders and the Horse, the writers explicitly said they were manifestations of the Smoke Monster. But that still begs the question "Man #2 and/or Smokey?"
Man #2 manifestations that seem obvious to me (because I endorse the 'body on Island' concept): Locke, Christian, Yemi, Boone, Alex, and Rousseau's Crew.
(Side Note. The sickness that Rousseau saw in her crew, I DO believe this to be Man #2. However, I think this is completely different than the sickness that Desmond and Kelvin were warned about. I think that was probably just radiation poisoning that they were worried about, considering the Hazmat suits they were wearing and the injections they were taking. It fits in considering that a nuclear bomb had once been detonated there; they were right on top of where the Incident had occured.) Sorry for the tangent, I had to :)
But I read a post by a guy named Curtis(on Facebook) and he proposed that Man #2 was actually 'eaten' by the Smoke Monster(remembering the line 'no thanks, I've just eaten'). Let's ponder on that for just a moment. Suppose that were true. Then all non-Hurley visions would most likely ended up being manifestations of the Smoke Monster (I would indeed like this if it were true, because it force the writers to explain exactly what the Smoke Monster is. But, alas, I do not believe that they are the same entity). But I don't think the form of Man #2 we saw is his TRUE form either. So, maybe we can throw that body on top of the pile of Man #2 manifestations. I don't know; can't say for sure right now but we'll go ahead and put him in that category.

Emily Linus: No clue. Her body was not on the island. She really screws things up when trying to find a pattern in the apparitions.
And worse is Claire. Her whole ordeal bugs the piss out of me. With her I especially think we need to see more of her story before we can make a decent speculation. I am inclined to think that she is dead since everyone who has appeared off-Island has been dead (referring to her appearance with Kate).
